  2. Endorsement from the Supervisor addressed to the Institute Director (for ZFF Staff)
  3. Duly accomplished Application Form
  4. Updated Curriculum Vitae (in .pdf file)
  5. BL and BL-related training certificates (scanned, in .pdf file)
  6. Portfolio with supporting evidence (if required)

REQUIREMENTS FOR RECERTIFICATION

Certification is valid for two (2) years, after which the trainer must submit a training hours log form showing the required hours of practice.
